2. On the Thank you page, answer “How did you hear about us?” and submit your response.
3. The same survey block can also appear on the Order status page. A completed survey stays completed for that order.

Please use example information and do not enter sensitive details in the survey.

In your own store, create a survey in OrderSurvey, then add and save the Post-purchase survey block on each page in Shopify’s checkout editor.
